Instructions

Before participating in any Carleton Archery event we require that you sign both the CUSA liability waiver as well as the Carleton Archery Acknowledgement of Risk and Club Information Form. Please read these forms carefully as they provide important safety information, outline our expectations of participants (such as following the safety rules), and they could impact your rights such as the right to sue. 

Please remember to fill out both forms, the forms and the submission forms are available at the link below.   

• On the next page, linked at the button below, you will find PDFs of the CUSA waiver to fill out.

• Completed PDFs can be submitted to submit@waiver.cuarchery.ca. 

• Once uploaded, click to the next page for the Carleton Archery form.

Please note: Carleton students under 18 are only required to complete the CUSA u18 liability waiver and only need to complete the Carleton Archery Acknowledgement of Risk and Club Information Form once they turn 18 if still participating in club events. So if you are under 18, you can skip step 2. 

• Once you have uploaded your CUSA waiver, click on the next button which will take you to the submission portal for the Carleton Archery Acknowledgement of Risk and Club Information Form. 

• The waiver text is provided as a drop down on that page, it can also be downloaded as a PDF for reference.

• We do not require a live signature for this form, your agreement will be signified by your completion of the online form.

Would you rather print and bring the completed forms to your first event? PDFs of all of the forms can be found here. You only need to submit the forms through one method, so no need to print the forms if you have already submitted them through the portal.
